Change to Agreement Records Forms
The following new forms enable the collection of the data required to update specific details on an Agreement Record in Themis Research and where appropriate, Themis Financials.
Note: making substantive, rather than administrative changes to an agreement
Please note: none of the below forms constitute a request for the approval of a change to the Agreement itself where prior approval is required by the sponsor and / or by the appropriately authorised University Officer. Except for changes to administrative details, approval by the sponsor and the University is almost certainly required before the following forms should be completed. If your desired change requires but is yet to receive the approval of the University and/or the sponsor, please contact either the Grants Office or the Research Innovation and Development Group Research immediately for advice, noting that you are enquiring about an Amendment.
You can do this by either calling #42000 or sending an e-mail request to themis-agreements@unimelb.edu.au.
